TERMS and CONDITIONS:
As a recipient of Los Padres Foundation (LPF) College Tuition Assistance (CTA) Program, I agree to
- Participate in LPF's mandatory five-day College Orientation Program during spring break.
- Forward to LPF copies of my transcripts, reports, and any other information regarding my school performance at the end and within the deadlines of each semester established by Los Padres Foundation.
- Send to Los Padres Foundation my official course registration documents. I understand that the two stipend installments (fall and spring semesters) will not be sent until LPF receives my official course registration documents.
- Volunteer 100 hours of my time to non-faith based community service during the academic school year. I understand that I will not receive that portion of my stipend until LPF receives proof of completion of the 100 hours of service.
- Maintain a minimum 3.0 GPA.
- Forfeit the remaining portion of the scholarship in the event that I discontinue the course of study stated in the application.
- Inform Los Padres Foundation of any changes in my address, phone number, e-mail or school.
- Apply the CTA stipend to defray education costs and expenses.
APPLICATION GUIDELINES:
The application must be completed ONLINE by JANUARY 16, 2016
All required documents must be postmarked no later than JANUARY 16, 2016
ALL APPLICATIONS MUST INCLUDE THE FOLLOWING:
1. Personal essay: Maximum two typed pages (total) covering topics A-E. (part of on-line application)
- Your future goals and objectives.
- Significant person(s) or event(s) that influenced your decision to pursue these goals.
- How your high school experience has influenced your personal growth.
- Why you are submitting an application to LPF College Tuition Assistance Program.
- Why you should receive a CTA stipend.
2. Letter of Reference: One letter identifying the student's strengths, skills, abilities, and special talents, and reasons why the student should he considered for a scholarship.
- Letter must be from your teacher, counselor, mentor, clergy, employer, etc.
3. Official Transcript. Must be sent by the school along with copies of SAT, PSAT, or ACT test scores.
4. Proof of income - Income tax returns, social security award letter, public assistance budget, pension award letter, etc.

REASONS FOR DISQUALIFICATION
- Failure to provide any of the items listed in 1, 2, 3, and 4 above.
- Any unanswered application questions.
- False information.
- Failure to attend the mandatory interview.
- Failure to attend and complete LPF College Orientation during spring break of your high school graduation year.
- Failure to present evidence of a high school diploma.
- Failure to register in an accredited college or university for the fall semester of your high school graduation year.
