As a recipient of Los Padres Foundation (LPF) College Tuition Assistance (CTA) Renewal Stipend, I agree to

1. Forward to Los Padres Foundation (LPF) copies of my Official Transcripts, reports, and any other information regarding my school performance at the end of each semester and within the deadlines established by Los Padres Foundation.
2. Send to Los Padres Foundation my official course registration documents. I understand that LPF will not send the two stipend installments (fall and spring semesters) until the foundation receives my official course registration documents.
3. Volunteer 100 hours of my time to non-faith based community service by June 1 each year. If I do not complete the volunteer community service requirement, I understand that I will forfeit $2,000. I will remain on probation until
   I complete 100 hours in the next academic year, maintain a minimum 3.0 Grade Point Average (GPA), and file a Renewal Application by June 1 of the next academic year.
4. Maintain a minimum 3.0 GPA. If my GPA is below 3.0, but I have filed my Renewal Application by JUNE 1 and completed the community service requirement, I will receive $500 for the fall semester If my GPA remains below 3.0, I will not receive $500 for the spring semester I will receive $1,000 if I complete 100 hours of volunteer community service by June 1.
5. Forfeit the remaining portion of the scholarship in the event that I discontinue the course of study stated in the application.
6. Inform Los Padres Foundation of any changes in my address, phone number, email, or school.
7. Apply LPF CTA stipend to defray education costs and expenses.
8. Send Official Transcripts and one (1) Letter of Recommendation.
9. Attend the LPF Reunion/Conference during spring break each year in NYC.

 

APPLICATION GUIDELINES:

The application must be completed ONLINE by JUNE 1, 2016

All required documents must be postmarked no later than JUNE 1, 2016

 

 

It is your responsibility to ensure that LPF receives all community service forms, your official transcript, and the reference by June 1

 

NO EXCEPTIONS.
