As a recipient of Los Padres Foundation (LPF) Second Chance Award, I agree to

1. Forward to Los Padres Foundation (LPF) copies of my transcripts, reports, and any other information regarding my school performance at the end of each semester and within the deadline established by Los Padres Foundation.
2. Send to LPF my official registration documents. I understand that LPF will not send the two stipend installments (fall and spring semesters, unless my program operates in a different time frame) until the foundation receives my official course registration documents.
3. Volunteer 100 hours of my time to community service during the academic school year. I understand that I will not receive that portion of my stipend until LPF receives proof of completion of the 100 hours of service.
4. Maintain a minimum 3.0 GPA. If my GPA is below 3.0, but I have filed my Renewal Application by JUNE 1 and completed the community service requirement, I will receive $500 for the fall semester If my GPA remains below 3.0, I will not receive $500 for the spring semester I will receive $1,000 if I complete 100 hours of volunteer community service by June 1.
5. Forfeit the remaining portion of the scholarship in the event that I discontinue the course of study stated in the application.
6. Inform Los Padres Foundation of any changes in my address, phone number, email, or school.
7. Apply LPF Second Chance stipend to defray educational costs and expense.

 

APPLICATION GUIDELINES:

The application must be completed ONLINE by JUNE 1, 2016

and your official transcript, and reference by JUNE 30, 2016

 

 

It is your responsibility to ensure that LPF receives all community service forms JUNE 1, 2016 and your official transcript, and reference by JUNE 30, 2016.
